One-year membership to Fort Wayne Pickleball Inc.

Welcome Pickleballer! By filling out and submitting this form, you agree to the following paragraph of stuff:

I am requesting to be accepted as a member and supporter of Fort Wayne Pickleball Inc. I recognize and understand that there are certain inherent risks to which I will be exposed because of the nature and level of the sports activity in which I have agreed to participate. I understand and agree that Fort Wayne Pickleball Inc, their agents and officials assume no responsibility for injury or illness I may sustain as a result of my physical condition or my participation in FWPI events. I understand it is my responsibility to provide my own accident and health coverage and that FWPI, their agents and officials, do not provide any accident or health insurance for their participants or volunteers unless notified at tournaments. I also give permission for the FWPI to use or distribute, without limitation or obligation, any record of the events which may include my voice or image. As evidenced by my submission of this form, I hereby, for my heirs, administrators and assigns, release, waive and hold harmless FWPI, their agents and officials from any manner of claims or lawsuits that may result from my participation in this sport. Annual Membership Dues are only $10.00 per person (plus $1.00 handling fee).  

All memberships expire at the end of the calendar year.

If you purchase your membership in the last three months of the year, it is good for the following year also.
